Where can I get CRB from?

Hi, my girlfriend is doing health and social at college and a year ago at the start of level 1 she was given a CRB leaflet to get sorted for this year. She seems to have misplaced it being a year ago and needs one to move up to level 2, but isnt rich so is there a place to request one from she is 17, can anybody help me?
